Homecare
The homecare team at Liverpool Heart and Chest Hospital provide the homecare service which delivers high-cost medications directly to patient’s homes whilst they are under the care of a hospital consultant. We currently provide medications to eligible cystic fibrosis and cardiology patients, and we work closely with our patients to provide an efficient service which enables patients to gain more control and manage their medications. We work alongside the clinical teams to initiate homecare medications for our patients and issue prescriptions to the homecare companies who then deliver the medications to the patient’s home or other suitable location.
As part of this service, we provide life-changing gene modulator drugs to many of our cystic fibrosis patients who now have a better quality of life with greater control over their symptoms. As a hospital we have played a role in the journey of these drugs, from participating in the initial clinical trials to now delivering them directly to our patients which is something we are proud of.
Within the team we have a pharmacist and pharmacy technician, and we regularly attend multi-disciplinary team meetings and outpatient clinics to introduce ourselves to our patients and explain the homecare service.
