What is Call 4 Concern?

Call 4 Concern (C4C) is a patient safety initiative designed to enable patients, families or carers to call for immediate help and advice if they feel concerned that the Healthcare Team has not recognised a change in their loved ones condition.

This patient and family centred approach will help us to improve all dimensions of quality and care including clinical effectiveness.

It will improve the experience of our patients and families in relation to the care and the treatment they receive in our hospital. Your call will go directly to the Critical Care Outreach Team (CCOT) whose main focus is to quickly respond to patients who are deteriorating in the hospital.

When should Call 4 Concern be used?

  • For patients who feel that their condition is deteriorating.
  • For a relative, friend or carer who has concerns about a hospital patient’s clinical condition.
  • If there has been a change in their clinical condition and they feel that the ward team are not addressing their concern.

The Call 4 Concern service should only be used if the nurse in-charge or team responsible for them has been spoken to and there are still concerns.

When should Call 4 Concern not be used?

  • Call 4 Concern is a patient safety initiative.
  • To report issues such as food, car parking, nursing care, discharge issues or any other general issues please contact the Ward Nurse, Ward Manager or Matron for the area.

What can you expect from the C4C service?

  • 24-hour access to an Outreach Team member who can prioritise your call and act up on your concerns in a timely manner.
  • A trained deterioration prevention specialist will assess you/your relative and discuss with the medical team and start any treatment changes and investigations that may be required.
  • Where necessary, they may refer you/your relative to a higher level of care to provide specialist advice and support.
