Request an Article or Book

Requests should be submitted using the form below.

You must be registered with the LHCH Library & Knowledge Service to use this service;
You will need your Library barcode number (located on reverse of your Trust ID badge) to request documents/ books;
Articles will be sent via PDF format to your LHCH email account (copyright permitting);
You may be asked to sign a copyright declaration form upon receipt of an article;
Article requests are usually supplied within 1-3 days. Book requests may take longer.

Please supply me with a copy of the material detailed above which I require for the purpose of research for a non-commercial purpose or private study. I declare that:

(a) I have not previously been supplied with a copy of the same material by you or any other librarian

(b) I will not use the copy except for research for a non-commercial purpose or private study and will not supply a copy of it, by any means, to any other person.

(c) To the best of my knowledge no other person with whom I work or study has made or intends to make, at or about the same time as this request, a request for substantially the same material for substantially the same purpose.

(d) If this item is delivered or was provided by an electronic method (including facsimile transmission), I will retain only a single paper copy and destroy any electronic copies after printing
