Patient Pass: Information for professionals
What is Patient Pass?
The system is designed to log emergency referrals for specialist advice that may require transfer between hospitals.
Patient Pass is a secure online platform accessible only via Trust devices. It enables instant two-way communication between clinical teams across hospitals and provides a central hub to document and track a patient’s treatment, management plan, and transfer where applicable.
This will NOT replace the Urgent Referrals Portal which will continue to be used to facilitate the transfer of patients.
Emergency requests for specialist advice will continue to be initiated by telephone, with referrers then required to log patient details on the system to ensure accurate recording of clinical information and to improve information sharing between referrers and LHCH.

Patient Pass can be accessed through via any web browser.
However, all users will have to set up an account using their GMC number and authenticate before access is granted to the platform.

Please use the relevant hospital Switchboard to contact the respective speciality.
However, ensure follow-ups are made via Patient Pass and the patient is added into the system.
Patient Pass is tested for 99% availability. However, business continuity procedures have been put in place:
- Immediate response - system status notifications sent to all users
- Backup procedures - revert to phone calls and secure email for urgent referrals
- Communication - clear escalation pathways for clinical teams.
