Patient Choice Award - who is your unsung hero?
Are you a patient or family member of a patient who has been to hospital or been treated by LHCH out in the community recently (since January 2016)? Did a member of staff 'shine' during your patient or family experience?
You can nominate a member of staff for the Patient Choice Award online now. Paper forms are available across the hospital in wards and departments or by printing the below form and sending to Joanne Shaw, Lead Nurse for Patient and Family Experience and Safeguarding, Liverpool Heart and Chest Hospital, Thomas Drive, Liverpool, L14 3PE.
Please note the closing date for nominations is 5pm on 14th October 2016.
The winners will be awarded at our annual LHCH Grand Awards 'Best of the Best' on Friday 2nd December so please get voting!
