CVD: Cardiac Rehab Services
Knowsley Community CVD Cardiac Rehab Service
For patients with cardiovascular disease (including Stroke, Cardiac, PVD and Heart Failure conditions) the Community Cardiac Rehabilitation Service for residents of Knowsley provides a number of rehabilitation choices in partnership with a multidisciplinary team of health professionals.
The aim of the Cardiac Rehabilitation Service is to improve the experience and outcomes for people who have been diagnosed with heart or vascular disease, ensuring they are supported in their return to as full an active life as possible, through lifestyle support clinics and comprehensive cardiovascular rehabilitation.
Patients, along with their partners, carers, or family members are encouraged and supported to achieve and maintain optimal physical health and social and psychological wellbeing.
The Community Cardiovascular Rehabilitation Team will:
- ensure that the patient experience is key in all aspects of the service and that patients understand their rehabilitation choices in supporting their recovery
- provide a safe, evidence based service to all patients in clinics, their own homes or community facilities
- provide advice, information and support to patient's carers and their families
- ensure all psychological, physical, social and environmental factors are assessed throughout the rehabilitation process improving quality of life
- advise of additional services that can support the patient and their family/carer in returning the patient to healthy lifestyle.
- ensure all rehabilitation services offered are accessible to all users.
