What do you need to do next?
Please contact the Patient & Family Support Team the next working day following your loved one’s death.
The team are available Monday - Friday 9.00am – 5.00pm (excluding bank holidays). A voicemail facility will be in operation if there is no one available to take your call, the team will call you back as soon as they can.
If the death has occurred after 4.00pm on a Friday you will need to contact the team the next working day which is the following Monday. The team will be able to discuss with you what happens next and when you may be able to collect the medical certificate of cause of death (MCCD).
When the MCCD has been completed by the doctor, the team will contact you and make an appointment for you to collect it from the hospital.
Please note that the MCCD will only be released to the nominated ‘next of kin’ documented within the deceased patient’s health record. If you have a nominated person attending on your behalf they will need a letter of authority and proof of identity.
The team will also arrange the return of any personal belongings such as clothing and valuables belonging to your loved-one. They will be able to talk you through the process of registering a death and make you an appointment to register at St Georges Hall, Liverpool.
The team will contact you as soon as the paperwork is ready and make you an appointment to attend the hospital. It is important that you wait for them to contact you before attending the hospital.
Medical Examiners System
The Medical Examiners System was introduced to the Trust from 1st September 2020.
The service offers a point of contact for bereaved families to raise concerns about the care provided prior to the death of a loved one with a Medical Examiner Officer or Medical Examiner.
This will take place in the form of a telephone conversation shortly after the deceased has passed away.
For further information about the Medical Examiners System
