Discharge Information
Liverpool Heart and Chest Hospital Discharge Team aid patients who require assistance with the discharge process.
The Discharge Team manage all of the simple and complex discharges from the Trust. They work closely with the Multi- Disciplinary Team arranging care packages, intermediate care beds, repatriations, organising transport, liaising with District Nurses and safe discharges for our patients.
Patients, relatives or carers who have any queries or issues post discharge should follow the following advice:
Up to 7 days post discharge contact the ward you were discharged from. Where a member of the nursing team will listen to your query and give advice.
Over 7 days seek advice from your GP doctor or 111 NHS advice line.

- Improve Discharge Planning Process
- Reduce numbers of patients being readmitted
- Improving the patient and family experience
- Ensuring patient flow is seamless
