Liverpool Heart and Chest Hospital Discharge Team aid patients who require assistance with the discharge process.
The Discharge Team manage all of the simple and complex discharges from the Trust. They work closely with the Multi- Disciplinary Team arranging care packages, intermediate care beds, repatriations, organising transport, liaising with District Nurses and safe discharges for our patients.
The Discharge Team also manage the Recovery Advice Line where patients, relatives and carers can call for information or advice within 30 days of discharge. After 30 days, please contact your GP.
A member of the nursing team will take your call, but if they are not available the patient’s name, number and a brief message should be left and the team will contact you as soon as they can.
